Requests for appeals of decisions by a HO (only if sanctions gi-o are imposed) are directed to the SADOS or designee. Requests for appeals shall be made in writing within five business days of the written decision. In cases involving multiple parties, all parties may file an appeal but as an individual party. If the written appeal is not filed on time it will be dismissed. A party is allowed only one appeal.
